Making an Appointment

Please call the office during our regularly scheduled hours (listed under Contact Us) to make an appointment.

For existing patients, we will need the following information to set up an appointment:

• Name of doctor
• Requested date of appointment
• Name of patient
• Phone number during the day
• Are you wearing contacts (rigid or soft) and/or eye glasses?
• Insurance provider and assigned insurance ID number

For new patients, we will need the following information to set up an appointment:

• Name of patient
• Requested date of appointment
• Home address
• Date of birth
• Home and business phone numbers
• Are you wearing contacts (rigid or soft) and/or eye glasses?
• Insurance provider and assigned insurance ID number
• New patients are encouraged to print, fill out and bring the Patient Information Form to their appointment to save time when checking in at the office.
